I AM NOT UNDER THE INFLUENCE OF DRUGS OR ALCOHOL.

I HAVE RECEIVED VERBAL AND WRITTEN AFTERCARE INSTRUCTIONS ADVISING ME OF THE PROPER CARE FOR MY TATTOO. ALL QUESTIONS ABOUT THE BODY ART PROCEDURE HAVE BEEN ANSWERED TO MY SATISFACTION.
